At Your Door Visiting Healthcare Services was started in 2015 by Angela Hoff, DNP, APRN, CNP. We have created our excellent reputation by providing compassionate care to the senior population in Ohio (and currently expanding to other locations) for a variety of assisted living facilities, independent living facilities, skilled nursing facilities, and memory care communities. Our professionalism, experience, and attention to detail, including highly responsive communication, have distinguished us from other visiting providers!

Our mission is to provide the highest quality care to all of our residents in order to allow them to age in place. We currently provide our services in Ohio and now the Tampa Bay Florida area!

Each At Your Door provider is board certified and able to care for patients of all ages. Our trained providers will come to your facility and utilize our expertise to provide your community with the same care expected at any doctor’s office.

At Your Door Visiting Healthcare Services is owned and operated by Angela (Angie) Hoff, DNP, APRN, CNP. She has been a Registered Nurse since 1993, with over 20 years in the home care industry. Angie has been a Nurse Practitioner since 2009 and has been providing primary care in a wide range of facility settings.

Angie received her MSN with a concentration in administration from Capital University in 1999. She received her post Master’s certificate and NP license from Otterbein University and finished her Doctorate of Nursing Practice from The University of Alabama in 2014. Angie is a Family Nurse Practitioner and specializes in geriatrics and dementia care. Her doctoral work focused on dementia and psychotropic drug treatments.

Sometimes it can be very difficult for your residents to get to their Primary Care Physician’s office. This is why Angie started At Your Door: Visiting Healthcare Services in 2015. With her passion for providing trusted, compassionate care, At Your Door has grown into one of the largest providers of “in residence” primary care services in the central Ohio area! Each of our providers is hand selected to continue Angie’s passion and love of our senior population!
